How to Become a DCS Vendor

If you recently finalized your adoption and you need to become a vendor with the Indiana Department of Child Services please follow these steps:

1. Review the New Vendor Instructions Form
2. Fill out the W-9 and Direct Deposit Authorization form per the New Vendor Instructions Form
3. Email completed forms to Centralized.Eligibility@dcs.IN.gov.
